Evidence Update
Current evidence
VITAL compared marine eicosapentaenoic acid/docosahexaenoic acid (EPA/DHA) supplementation at 1 g/day with placebo within a factorial trial of 25,871 adults in primary prevention. Its vitamin D3 assignment belongs to the same randomization, not a separate replication. VITAL's marine omega-3 supplement did not significantly reduce its primary major cardiovascular endpoint in general primary prevention.
What changed
VITAL's primary MACE comparison was null; its MI result is secondary and does not establish the REDUCE-IT population's benefit.
What did not change
Cross-trial differences cannot identify whether formulation, comparator or risk caused the different results.
Why it matters
Prevents fish-oil class claims and preserves formulation-specific adverse tradeoffs.
Main limitation
Secondary MI finding does not reverse primary result; not equivalent to high-dose prescription EPA or proof of no effect.
Next proof
A direct randomized comparison of specified formulations and comparators in the same risk population, with prespecified cardiovascular and safety outcomes.
