Evidence Update

Current evidence

Look AHEAD compared intensive lifestyle weight loss and physical activity with diabetes support and education in adults with type 2 diabetes. Its cardiovascular and mobility analyses come from the same randomization. Look AHEAD's intensive weight-loss lifestyle programme did not significantly reduce the primary cardiovascular composite in adults with type 2 diabetes. Look AHEAD's functional analysis found less mobility loss with intensive lifestyle intervention despite the separate cardiovascular null result.

What changed

Look AHEAD's mobility and cardiovascular reports use the same randomization; their different endpoints can legitimately have different conclusions.

What did not change

Mobility odds are not cardiovascular risk, survival or a universal weight-loss verdict.

Why it matters

Gives customers an outcome-specific healthspan interpretation.

Main limitation

Not equivalence, not a denial of other functional benefits. The secondary-composite correction adds angina hospitalization to Table 2 secondary-composite reporting; primary effect unchanged. Self-report and modeled states; OR is not an absolute risk reduction or mortality effect. Slightly more year-1 musculoskeletal symptoms.

Next proof

Independent confirmation of durable functional outcomes and separately prespecified cardiovascular outcomes.

Sources

  1. Cardiovascular Effects of Intensive Lifestyle Intervention in Type 2 Diabetes [Correction]

Source type formal_correction Publication date 2014

What this source establishes Corrects the Look AHEAD secondary-composite label to include angina hospitalization.

Important limitation The corrected label does not change the primary cardiovascular effect or create a new trial.

Evidence access Relevant full-text sections reviewed.

  2. Cardiovascular effects of intensive lifestyle intervention in type 2 diabetes.

Source type primary_research_publication Publication date 2013

What this source establishes Reports the Look AHEAD primary cardiovascular comparison and futility stopping.

Important limitation A null primary result is not equivalence and does not negate separately measured functional outcomes.

    Evidence access Abstract reviewed.

  3. Lifestyle change and mobility in obese adults with type 2 diabetes.

Source type primary_research_publication Publication date 2012

What this source establishes Reports modeled mobility limitation from the Look AHEAD cohort.

Important limitation Self-reported mobility and an odds ratio are not absolute cardiovascular risk reduction or a survival effect.

    Evidence access Abstract reviewed.
