Evidence Update

Current evidence

In TRAILBLAZER-ALZ 2, donanemab slowed decline compared with placebo in people with early symptomatic Alzheimer disease and brain-imaging evidence of amyloid and tau. Both groups still declined. Brain swelling was substantially more common with donanemab, and some deaths were considered treatment related. This randomized trial did not demonstrate restored cognition, prevention in people without symptoms, or comparative benefit over another Alzheimer drug.

What changed

Donanemab slowed clinical decline versus placebo in early symptomatic Alzheimer disease, with substantial ARIA risk and decline in both groups.

How to read these findings

ARIA-E refers to amyloid-related imaging abnormalities involving brain swelling or fluid accumulation. The phase-3 swelling and treatment-related death figures below belong to this randomized trial, not later pooled or open-label studies.

An estimand specifies the effect being estimated in a particular analysis population. Tau-defined patient groups and allocated statistical significance thresholds must not be combined as though they were one analysis.

The held later safety work is separate: the internal C33 reference below is a boundary, not an additional accepted source. Slower decline is not restoration, prevention in people without symptoms or superiority to another Alzheimer drug.

Accepted limitations and study details

Exact reviewed qualifications are retained below. These details do not add a new accepted claim.

Randomized phase-3 details — later pooled/open-label work excluded
  • Different tau estimands/allocated alpha; not cognitive restoration, asymptomatic prevention or a comparison against lecanemab.
  • Design: Randomized, double-blind, placebo-controlled phase 3 TRAILBLAZER-ALZ 2 trial only.
  • Safety: Phase 3 ARIA-E 24.0% versus 2.1%; 52 symptomatic donanemab cases; three versus one deaths considered treatment related.
  • Interpretation: Clinical slowing is not reversal; C33’s later pooled/open-label safety work is separate and is not part of this randomized comparator.

Why it matters

These findings apply to the studied populations, regimens and outcomes; they do not establish generalized longevity benefit.
