Mediterranean dietary pattern with olive oil or nuts

Evidence Update
Current evidence
A Mediterranean dietary pattern supplemented with extra-virgin olive oil or nuts was compared with control dietary advice in PREDIMED among 7,447 adults at high cardiovascular risk. PREDIMED's withdrawn 2013 report must not serve as the current efficacy source; the 2018 replacement is a reanalysis of the same experiment after randomization deviations. PREDIMED's replacement analysis found lower major cardiovascular events with supplemented Mediterranean dietary patterns, with randomization deviations remaining material.
In a selected body-composition group from PREDIMED-Plus, adults aged 55–75 received either a calorie-reduced Mediterranean diet with activity and behavioural support or advice to follow a Mediterranean diet without calorie restriction. The combined programme improved relative fat and lean-mass proportions, but absolute lean mass still fell. A higher percentage of lean mass is not muscle gain, and these measurements do not demonstrate fewer clinical events. Population: PREDIMED-Plus body-composition subset 1521 of 6874, age 55–75, mean 65.3, 52.1% men; intervention 760/control 761. This is not the separate 4746-person diabetes-incidence report. Species: Homo sapiens Intervention and comparator: Energy-reduced Mediterranean diet plus physical activity and behavioral support Comparator: Advice to follow ad libitum Mediterranean diet without physical-activity promotion Endpoint: Intermediate DXA outcomes, completer analysis with imputation sensitivity Reported result: 760/761; year-3 differences: fat −0.38 percentage points (−0.64 to −0.12), lean +0.34 (0.09–0.60), visceral fat −70.4 g (−126 to −15.2). Follow-up: 3 years body composition; 6 years diabetes report
What changed
The replacement analysis supports a bounded supplemented dietary-pattern cardiovascular finding, not an isolated olive-oil or nut effect.PREDIMED-Plus improved relative body composition versus ad libitum Mediterranean-diet advice, but a larger lean-mass percentage did not mean absolute lean-mass gain.
What did not change
The 2013 and 2018 articles cannot be counted as independent replication.Selected DXA centers/completers and a combined program; not clinical-event proof or isolated diet effect. Absolute lean mass fell.
Why it matters
Preserves a material benefit alongside its corrected provenance and design limitations.
Main limitation
Replacement does not erase deviations or create independent replication. Original retained only as historical provenance. Adjusted baseline/propensity methods and sensitivity exclusions support bounded interpretation, not an unqualified pristine-RCT claim or a causal single-food effect.Selected DXA centers/completers and a combined program; not clinical-event proof or isolated diet effect. Absolute lean mass fell. Design: Randomized multifactorial lifestyle program; interim DXA subgroup; separate secondary diabetes report Safety: Absolute lean mass declined with weight loss; comprehensive safety not established by body composition. Interpretation: PREDIMED-Plus is not PREDIMED; inaccessible 2026 correction; percent lean versus absolute lean.
Next proof
Independent well-randomized replication of the dietary-pattern comparison, with transparent assignment and clinical cardiovascular outcomes.Prespecified clinical outcomes and functional effects.
