Evidence Update

Current evidence

Daily multivitamin-mineral supplementation was compared with placebo for cognitive performance in COSMOS ancillary studies; the pooled analysis included 5,203 nonoverlapping participants from one programme. COSMOS ancillary studies support a small multivitamin cognitive-performance signal, not demonstrated dementia prevention or three independent trial replications.

What changed

COSMOS pooled ancillary results suggest a small cognitive-performance benefit; they do not demonstrate dementia prevention or three independent programme replications.

What did not change

Small score shifts or age-equivalent calibrations are not demonstrated rejuvenation.

Why it matters

Separates ingredient attribution, endpoint hierarchy and participant denominators.

Main limitation

Selected subsets, small score differences, different assessment modalities. Cross-sectional age-equivalent calculations are not years of biological rejuvenation.

Next proof

Independent programme with clinically meaningful function/dementia outcomes and durable follow-up.
